The Phoenix Medical Practice
The Phoenix Medical practice is a small, friendly, well established single-handed undergraduate teaching practice. Dr Khan is the principle GP and the named accountable GP for all patients registered at the Practice. The practice is proud to offer the highest standard of patient centred healthcare. We run many clinics for the management of chronic disease (eg, asthma, CHD, diabetes) and offer a wide variety of other medical services including minor surgery, childhood vaccinations and well person checks.
We are a member of the Family Doctor Association (FDA). The Family Doctor Association leads the way to the future for grassroots primary care. With over 1000 members inspired for over 30 years. Continuity of care benefits patients, improves health outcomes and saves the NHS money. The member GP practices aim to provide high quality medical care while maintaining the traditional personal care of the family doctor in a friendly environment promoting health by education and preventative measures.

To enable access to the surgery for disabled patients we have the facility of a ramp and double doors for easy access. The practice is based on ground floor level only.
CQC (Care Quality Commision)
The Care Quality Commission (CQC) make sure that hospitals, care homes, dental and GP surgeries, and all other care services in England provide people with safe, effective, compassionate and high quality care and encourage them to make improvements.
The CQC do this by inspecting services and publishing the results on their website to help to make better decisions about the care you receive.

Why Do People Miss Hospital Appointments?
Healthwatch Doncaster has commenced a joint campaign into missed hospital appointments and has launched an online survey.
They are keen to gather feedback as widely as possible. Thank you for taking time to complete the survey.
This is a free on-line service that can provide you with:
- On-line booking, checking and cancelling of appointments
- On-line ordering of repeat medication
- Ability to change your contact details
It is available 24 hours per day, 7 days a week via any computer with an internet connection. It means that you don’t need to ring reception for these services and that you can arrange them at a time to suit you. It is also free for you to create an account and use the service. To register for the service please speak to the receptionist for your own personal user name and password (photographic identification may be required).
Systmonline is also available to patients as an app for iphones, ipads and othe apple devices running the iOS operating system. The app is listed on the App Store and is free to download. The app is now also available for android devices
If there is a problem with accessing your account please do not hesitate to call the surgery and speak with a member of staff and they will try and resolve any problems.
